City Folk Privacy Policy
Scope of this Policy
This policy applies to “Consumer Health Data” as defined by the Washington State My Health My Data Act (MHMDA). “Consumer Health Data” under MHMDA means personal information that is linked or reasonably linkable to a consumer and that identifies the consumer's past, present, or future physical or mental health status.
This Policy does not apply where an exception or exemption applies such as for Protected Health Information (“PHI”) under the Health Insurance Portability and Accountability Act of 1996 (“HIPAA”).
Collection of Consumer Health Data
While we don’t intentionally collect Consumer Health Data, AK Creative LLC dba City Folk may come into possession of Consumer Health Data in connection with your use of our services. For example, you may choose to share this type of data with us when you sign up for a class, workshop, event, or purchase a product we sell. We may also use Consumer Health Data for other purposes where you provide us consent to do so or as otherwise permitted by law.
Examples of Consumer Health Data may include information such as:
a. Information about health-related conditions, symptoms, diagnoses, or treatments that you provide to us through intake forms, surveys or other communication regarding community health (ex. reaching out to us about accessibility needs or illness)
b. Photographs, which may be considered biometric information under the MHMDA
c. Information that could identify your attempt to seek health care services or information, including services that allow you to assess, measure, improve, or learn about your or another person’s health.
d. Other information that may be used to infer or derive data related to the above or other health care information.
Your Consumer Health Data may also be collected via third-party systems and platforms we use, for example: Luma, Squarespace, Venmo, Google, Flodesk and payment platforms. These third parties may have their own privacy policies which apply to the collection and use of your Consumer Health Data.
Use of Consumer Health Data
AK Creative LLC dba City Folk collects and uses consumer health data as reasonably necessary to provide you with the services you have requested or authorized. This may include: Your name, address, payment information, requests per our Masking and Accessibility Policies.
We may use consumer health data for other purposes for which we give you choices and/or obtain your consent as required by law – for example, for advertising or marketing purposes.
How to Exercise your Rights
